    • Pre-natal Consultation
      • (Every Friday) 1:30 PM – 5:00 PM
    • Health Education Service
      • Monday-Friday (except on Holidays) 8:30 AM-12:00NN
    • Minor Surgery (non-emergency/ambulatory clinical surgery)

  • MC for routine medical/physical exam for employment purposes; scholarship;driver’s license; OJT for VSU students
    • Laboratory results – CXR, CBC, U/A, S/E and other laboratory tests (may be requested as specifically required by the physician)
    • Official Receipt
  • Medico-Legal Certificate
    • Request for issuance of medico-legal certificate from police department
    • Official Receipt
  • Annual Medical Exam. Certificate for VSU faculty and staff
    • Laboratory results (Chest Xray, CBC, Urinalysis)
    • Official Receipt
  • Medical Examination for Freshmen and Transferees.
    • Medical exam referral from the Admissions Office for College students; from Principal for High school students
    • 1 copy 1x1 ID picture
